FIRE SERVICE PROVIDER REINSTATEMENT
Volunteer Firefighter, Firefighter I, Firefighter II, Fire Safety Inspector and Hazard Recognition Officer

Expired or surrendered certificates are reinstated in accordance with 4765-20-08 of the Administrative Code.

Revoked certificates are reinstated in accordance with 4765-20-09 of the Administrative Code.

Reinstatement requirements are determined by time elapsed since expiration. A $75.00 application fee (check/money order to “Ohio Treasurer of State”) is required with the reinstatement application.

To reinstate a certificate an applicant must have previously have held a certificate, at the level of certification sought, that was in good standing at the time the certificate expired.

No More Than 90 Days Since Certificate Expiration

Note: This option is ONLY for those who completed the continuing education requirements BEFORE the certificate expiration date AND if the certificate has been expired for less than 90 days.

Do not submit reinstatement application until all requirements are met.

  1. Provide proof of CE completed during the certification cycle (cannot be completed after expiration date). Proof must have signature of applicant’s fire chief or program director of an Ohio chartered fire training program.
  2. Attach proof of CE to completed reinstatement application.

18 Months or Less Since Certificate Expirations

Do not submit reinstatement application until all requirements are met.

  1. Provide proof of CE for the last certification cycle signed by the applicant’s fire chief or program director of an Ohio chartered fire training program. CE can be completed after the certificate’s expiration date.
  2. Pass the knowledge examination at your level of certification with a 70 percent or better. If the exam is not passed within 3 attempts, the course of training must be repeated.
  3. After passing the knowledge exam, attach proof of CE signed by the applicant’s fire chief or program director of an Ohio chartered fire training program to your completed reinstatement application.

More than 18 Months but not more than 36 Months Since Certificate Expiration

Do not submit reinstatement application until all requirements are met.

  1. Provide proof of CE for the last certification cycle signed by the applicant’s fire chief or program director of an Ohio chartered fire training program. CE can be completed after the certificate's expiration date.
  2. Pass the practical examination at your level of certification with a 70 percent or better. If the exam is not passed within 3 attempts, the course of training must be repeated.
  3. Pass the knowledge examination at your level of certification with a 70 percent or better. If the exam is not passed within 3 attempts, the course of training must be repeated.
  4. After passing the practical and knowledge exams , attach proof of CE signed by the applicant’s fire chief or program director to your completed reinstatement application.

More than 36 Months Since Certificate Expiration

  • The applicant is not eligible for reinstatement and the course of training must be repeated.

Acceptable Proof of CE

  • CE Completion Certificate(s) individually signed by the applicant’s fire chief or the program director of an Ohio chartered fire training program; or
  • Training records, if such records include the CE class name, completion date, and respective CE hours. Each page of the training records must be signed, dated and approved for purposes of reinstatement by applicant’s fire chief; or
  • Letter, signed and dated, from the applicant’s fire chief or the program director of an Ohio chartered fire training program, that details the CE class name, completion date, and respective CE hours for purposes of reinstatement.
