Fire Service Instructor Certification Audits
(Firefighter & Fire Safety Inspector Instructors)

The second page of the audit form includes “Instructor Audit Form Instructions” for responding to an instructor audit. It is important to read and follow the directions carefully.

You need to list all continuing education courses you taught AND instructional continuing education you attended during the audited certification cycle.

Fire Instructor

  1. Classes taught must be taught through an Ohio chartered fire training program or through in-service fire CE training. Classes taught must be approved by the certificate holder’s fire chief or a program director from an Ohio chartered fire training program.
  2. Instructional CE hours must be related to fire service training as approved by the certificate holder’s fire chief or a program director from an Ohio chartered fire training program.

Fire Safety Inspector Instructor

  1. CE must be relevant to fire safety inspector objectives and approved by the certificate holder’s fire chief or a program director from an Ohio chartered fire training program.
  2. Instructional CE hours shall be related to a fire safety inspector instructor’s training as approved by the certificate holder’s fire chief or a program director from an Ohio chartered fire training program.

You have two options for submitting documentation for the hours claimed:

  1. Attach supporting documentation (certificates of attendance)
  2. Attach a computer-generated training record from your employer

When submitting a computer-generated training record from an employer, the training record must include the training site charter number or F.D.I.D. number, and a signed and dated attestation from the program director or the certificate holder’s fire chief.

Only continuing education and instructional hours obtained during the audited certification period will be accepted.
