Expired or surrendered certificates are reinstated in accordance with 4765-21-07 of the Administrative Code.
Revoked certificates are reinstated in accordance with 4765-21-08 of the Administrative Code.
To reinstate a Live Fire Instructor certificate:
If a Live Fire Instructor certificate has been expired or becomes invalid due to voluntary surrender, for two years or more from the expiration date or date of surrender, the applicant shall complete a new "Live Fire Training Operations Course" and "Live Fire Training Operations Job Performance Requirement Check-Off," and meet all requirements for a Live Fire Instructor certificate as set forth in rule 4765-21-03 of the Administrative Code. The applicant must apply as a new applicant.
A holder of a current and valid Fire Instructor or Assistant Fire Instructor certificate, exempt ("grandfathered") from Live Fire Training Operations Course requirements shall no longer be exempt from the requirements to complete the "Live Fire Training Operations Course" and the "Live Fire Training Operations Job Performance Requirement Check-Off" if the Fire Instructor or Assistant Fire Instructor certificate has lapsed since April 7, 2014. The applicant shall complete a new "Live Fire Training Operations Course" and "Live Fire Training Operations Job Performance Requirement Check-Off," and meet all requirements for a Live Fire Instructor certificate as set forth in rule 4765-21-03 of the Administrative Code. The applicant must apply as a new applicant.
Assistant Fire Instructors are not eligible for reinstatement. Expiration or surrender of an Assistant Fire Instructor certification results in loss of a Live Fire Instructor certification. An Assistant Fire Instructor certificate shall be reacquired prior to being eligible for the Live Fire Instructor certification. Upon reacquisition of the Assistant Fire Instructor certificate the candidate shall complete a new "Live Fire Training Operations Course" and "Live Fire Training Operations Job Performance Requirement Check-Off," and meet all requirements for a Live Fire Instructor certificate as set forth in rule 4765-21-03 of the Administrative Code.
NOTE: If a Fire Instructor who held a Live Fire Instructor certificate achieved through successful completion of the "Live Fire Training Operations Course" and "Live Fire Training Operations Job Performance Requirement Check-Off" (non-"grandfathered"), that was in good standing at the time of expiration or voluntary surrender, is reinstated by the executive director, the Live Fire Instructor certificate shall automatically be reinstated if the certificate has been expired or surrendered for a period of less than two years.
