Update Contact Information

In accordance with the Ohio Administrative Code, EMS and fire certificate holders shall notify the Division of EMS no later than 30 days following any changes in:

  • Full legal name
  • Home mailing address
  • Home (primary) telephone number
  • Email address

The contact information is necessary to notify certificate holders of certification expiration and renewal requirements.

Notification can be done online, or you may download, print, complete, and mail or fax the form to the Division of EMS:
