MTLS Online System

MTLS Online is our online system to apply for, renew, and manage your medical transportation license. We no longer accept paper applications and forms. All fees must be paid via credit card or Automated Clearing House (ACH).

First-Time Users

First-time MTLS Online users must register with the Ohio Department of Public Safety Identity Manager:

  • Select the “Continue” button on the landing page.
  • Once open, click on “Register” at the bottom of the box to start the process of setting up your account.
  • After registration with Identity Manager, you will receive an email from MTLS granting you access to the MTLS Online system. This may take up to 24 hours

Already Have an Account?

If you have already registered with Identity Manager, you can access the MTLS Online System by signing in with your email address and password.
